从2015年3月1日起，浙江工业大学留学生办理签证延期，需在原材料基础上，增加中华人民共和国浙江出入境检验检疫局认证或组织的体检报告。

Notice for extending visa:

From Mar 1st 2015, if you want to extend your visa, besides the materials and documents which are required now, you have to take PHYSICAL EXAMINATION RECORD FOR FOREIGNER which is taken or certified by Zhejiang Entry-exit Inspection and Quarantine Bureau

Address for the Bureau: No.230 Zhonghezhong Road,Hangzhou

受理时间：星期一至星期五8:00-11:00am

Time of physical exam: 8:00–11:00am from Monday to Friday.

请您携带：1. 本人身份证或护照原件

2.二寸近期证件彩照（出入境人员3-6张；入境人员4-5张）。

Need to bring: passport and 4 colored photos (size: 3.5×4.5cm).

温馨提示：体检早晨需要空腹。女性请避开经期，孕妇需要告知咨询医师。

Note: No breakfast. Not in menstrual period. Pregnancy should be known by the doctor.

取证时间：第二个工作日15:00-16:30am.请凭取证凭单领取证书。

Time of getting certificate: The 2nd working day 3:00pm–4:30pm. You have to take the receipt that is given by the doctor with you to get the medical report.

费用：持6个月有效期内的外国人体格检查表(Foreigner Physical Examination Form)做认证，费用为196人民币；否则，基础检查套餐费用为440人民币。

Fee:196RMB is for the certification fee in the bureau for the Foreigner Physical Examination Form you already have to get it certified in the bureau and the form has to be no longer than 6 months until the date you go to the bureau. Or 440RMB to do the medical check in the bureau again.
